26-27 Adult Supervision, Medical Release & Video/Photo ReleaseAdult SupervisionParents/guardians are responsible for the supervision and safety of their child at all times while on-site or at any MPSD-related location, except during the dancer’s scheduled class time when the child is participating as a registered student.Once class has ended, responsibility for the child returns to the parent/guardian. Parents/guardians are expected to pick up dancers promptly at the end of class.Medical ReleaseIn the event of an emergency, Major Productions Studio of Dance (MPSD) will make every effort to contact the parent/guardian using the phone numbers listed on the dancer’s account.Parents/guardians are responsible for keeping all contact and medical information current and accurate. Updates may be made at any time through the Parent Portal.If a parent/guardian cannot be reached during an emergency, I authorize the supervising MPSD staff member to obtain appropriate medical care for my child. This may include treatment by a physician, hospitalization, anesthesia, surgery, medication, or any other care deemed necessary by medical professionals.I understand that I am responsible for any costs related to emergency medical treatment. I also confirm that the information listed on my child’s account is current and accurate to the best of my knowledge.Video & Photo ReleaseI grant Major Productions Studio of Dance permission to photograph and/or video my child during classes, rehearsals, performances, events, competitions, and other MPSD-related activities.I understand that these photos and videos may be used by MPSD for promotional, informational, and non-commercial purposes, including but not limited to:Social mediaMarketing materialsStudio websiteFlyers or advertisementsEmails or newslettersCommunity Social PagesStudio displaysI understand that my child’s image may be used without compensation, and I waive any right to payment, ownership, or approval of the final materials used by MPSD.
