Liability Release

By registering and/or participating in activities with Major Productions Studio of Dance (MPSD), the parent/guardian acknowledges and agrees to the following:

Acknowledgement of Risk

Participation in dance classes, rehearsals, performances, events, and competitions involves physical activity and carries inherent risks. These risks may include, but are not limited to, physical injury, strains, sprains, fractures, falls, and other possible injuries or medical conditions.

Voluntary Participation

I understand that participation in MPSD activities is voluntary. I voluntarily choose to participate and/or allow my minor child to participate in dance-related activities organized by Major Productions Studio of Dance, Samuel and Tracy Stratman, their trustees, agents, independent contractors, employees, and instructors.

Assumption of Risk

I understand and accept all risks associated with participation in dance classes, rehearsals, performances, events, and competitions. This includes activities held at Major Productions Studio of Dance and any off-site locations, including but not limited to:

• Sarasota County Schools

• Manatee County Schools

• Imagine Schools

• Ellenton Ice and Sports Complex, 5309 29th Street East, Ellenton, FL 34222

• Any other location where MPSD classes, performances, events, or competitions may take place

Release of Liability

In consideration of being allowed to participate in MPSD activities, I, on behalf of myself, my minor child, my heirs, executors, administrators, and assigns, release and hold harmless Major Productions Studio of Dance, Samuel and Tracy Stratman, their trustees, agents, independent contractors, employees, instructors, Sarasota County Schools, Manatee County Schools, Imagine Schools, Ellenton Ice and Sports Complex, and any other associated entities, facilities, or locations from any and all claims, demands, damages, losses, actions, or causes of action related to injury, illness, damage, or loss that may occur as a result of participation in MPSD activities.

Medical Authorization

In the event of an injury, illness, or emergency, I authorize Major Productions Studio of Dance and its representatives to obtain necessary medical treatment for myself and/or my minor child. I understand and agree that I am responsible for any medical expenses or costs related to such treatment.

Indemnification

I agree to indemnify, defend, and hold harmless Major Productions Studio of Dance, Samuel and Tracy Stratman, their trustees, agents, independent contractors, employees, instructors, associated entities, and activity locations from any claims, damages, losses, costs, or expenses, including reasonable attorney fees, arising out of or related to my or my minor child’s participation in MPSD classes, rehearsals, performances, events, or competitions.
